IMPORTANT INSTRUCTIONS FOR FILLING ON LINE AND MANUAL FORM 

1. Candidates to fill in the  online form as well as submit a manually filled form in the school, along with an application fee of Rs.500/-. The manual Form can be downloaded from the link given below or can be collected from the school reception.

2. On filling out the online form, a registration code will be generated. This registration code is to be filled in the space provided on the top of the form as per the given example.

   

3. The percentage should be filled correctly up to two decimal places (Example 78.89). No other symbol like % is needed. Correct entry is 78.89, incorrect entry is 78.89%.
4. If you have appeared in any exam and the result is awaited, leave the percentage column empty.
5. Fill the two main teaching subjects of B.Ed. (Example:- Social Science, English).
6. Leave the percentage column of CTET blank if you have not qualified for the exam.
7. Check your form carefully before submitting it, as after submitting nothing can be changed and no warning will be issued.
8. The information on the manual form must match the information provided on the online form.
9. The manual Form must be accompanied by the attested copies of all the certificates mentioned in the form.

Attach a copy of the DAV CBT Provisional Eligibilty Card (P.E.C.) with the QR Code for verification.
